Traditional RACI matrices assume one system, one owner. Multi-agent chains break that assumption: an error three steps downstream may trace back to an upstream agent's bad output. This matrix assigns accountability across the full chain, not just per agent in isolation. Informed by AI Act Art. 9 (risk management) and DAMA-DMBOK v2.
